What to Bring to your Visit

List of Active Medications
Medical Records- please bring any of the following if relevant to your visit
Prior Imaging Studies (XRs, MRIs, CT scans)
Radiology reports can be of great value. In addition, a disc of images allows Dr. Brusalis to personally review and interpret your imaging studies. We greatly appreciate every effort to bring an imaging disc with you to your visit.
Prior operative reports and/or clinic notes
MyHSS Portal
HSS strives to minimize clinic waiting time by streamlining the process for patients to provide their information prior to your appointment. We encourage you to set up your MyHSS account and provide our office with your information online. This will significantly reduce your time completing paperwork before your first visit.
